4. Proof of work or earnings
      a) Company check with federal withholdings
      b) Canceled checks for the last two months (if you are self-employed)
      c) Last W2 (if you are self-employed)
      d) Letter of proof of employment describing salary and time with the company
          (must be printed on official company letterhead, NO COPIES please)
